TO THE EDITOR:
In "Severe Open Subtalar Dislocations. Long-Term Results" (77-A: 10751079, July 1995), Goldner et al. classified open dislocations according to the commonly used system of Gustilo et al.2-4. To my knowledge, this classification (or, perhaps more accurately, grading) system was developed for open fractures and not for the treatment of open joint injuries. The only exception was an open dislocation with arterial disruption that was classified as a type-IIIC lesion in a 1990 review by Gustilo et al.4.
There is no argument with the application of the system of Gustilo et al. to open fractures or dislocations of a joint. As far as I can tell from my reading of the literature, there has been no groundwork done to validate or substantiate a classification system that is equally applicable to open joint injuries.
