Aneurysmal bone cysts are benign tumors that arise most often in the shaft of a long bone and occur frequently in young patients. These lesions usually are characterized by painful swelling and have an expansible, honeycombed appearance on radiographs. Aneurysmal bone cysts in the cephalad portion of the spine are rare1,9,12. This is the first report, to our knowledge, of an aneurysmal bone cyst in the lateral mass of the atlas that was treated operatively through a lateral approach.
