A patient with thoracic outlet syndrome secondary to an anomalous first rib forming a pseudarthrosis with the second rib is presented. A large infraclavicular mass was present which caused partial arterial compression and complete venous compression when the arm was abducted or flexed. This is believed to be the first report of venous occlusion associated with this particular bone abnormality.
